Use it the same day
No waiting periods and no claim forms. Your effective date stays on file with our office — no membership card needed.
Save all year
Discounts are applied automatically at checkout. Renew on time and save another 5% with auto-renewal.
PROGRAM GUIDELINES
- Patient's portion of the bill is due the day of service.
- $50 reinstatement fee applies if your plan lapses.
- Cannot be used in conjunction with another dental plan.
- CareCredit may not be used to pay a D$P premium.
- No refunds of premiums are issued at any time — non-refundable.
- This plan is only honored at Grand Mission Dentistry.
EXCLUSIONS & LIMITATIONS
This program is a discount plan, not a dental insurance plan. It cannot be used:
- For treatment which, in the sole opinion of our doctors, lies outside their scope.
- For referrals to specialists.
- For hospitalization or hospital charges of any kind.
- For costs of dental care covered under automobile medical.
- For services or injuries covered under workers' compensation.
